Job summary

TruHealth seeks an advanced practice provider (NP/PA) with geriatrics focus to join our long-term care team. You will collaborate with physicians, nurses, and case managers to deliver coordinated primary care in nursing facilities and assisted living communities.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess medical and mental health needs; create a proactive plan of care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics, and consults.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
