Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with experience in primary care or internal medicine, preferably with geriatrics focus. This position gives you the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers.

Qualifications

• License as FNP/AGNP/AHACNP or PA is required.
• Master's degree or higher from an APRN/PA program.
• 3 years of clinical nursing or rehab experience in geriatric populations.
• Experience with Electronic Health Records and MS Office.

Responsibilities

• Assess patients and plan proactive care.
• Ensure regulatory compliance for clinical services.
• Prescribe medications and order labs/diagnostics.
• Monitor treatment and adjust plans.
• Collaborate with RN case managers within an integrated care team.
