Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with a geriatrics focus to deliver coordinated primary care within nursing homes and assisted living facilities.

You will work with a team of providers and case managers to improve quality of life and reduce hospital visits while adhering to federal and state regulations. Strong EHR and Microsoft Office skills are essential.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess residents’ medical and mental health needs and create proactive care plans.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor treatment progress and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
