Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care or internal medicine, preferably with geriatrics focus. This position gives you the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers.

Qualifications

• Masters degree in nursing (APRN) or physician assistant program.
• APRN/PA license required for practice.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Experience with Electronic Health Records.
• Proficiency in Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients' medical and mental health needs and create a proactive care plan.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
