Job summary

NYC Health + Hospitals Coler is seeking a Nurse Practitioner for geriatric primary care at the Roosevelt Island facility. You will provide medical management for residents, perform medical assessments, and collaborate with attending physicians to optimize patient outcomes. Requires NYS NP certification and BLS.

Position includes leadership in QA/PI initiatives, discharge planning, and coordination of rehab and ancillary services within a high-quality care environment.

Qualifications

• A valid New York State license and current registration to practice as a Registered Professional Nurse (NYSED).
• Current Nurse Practitioner certification issued by NYSED.
• BLS certification through the American Heart Association.
• Long-term care or geriatric experience.
• Experience with Post-Acute EMR.
• Familiarity with CMS and NYS DOH regulations regarding medical management of nursing facility residents.

Responsibilities

• Provide medical management for geriatric and non-geriatric nursing facility residents including primary care.
• Write orders for medications, treatments, diagnostic studies and other necessary care.
• Collaborate with attending physicians on assigned patient cases.
• Coordinate discharge planning and transitions of care for residents.
• Provide medical leadership for Quality Assurance and Performance Improvement initiatives.
