Job summary

Gobravvo in Tucson, AZ is seeking a Nurse Practitioner to provide comprehensive outpatient primary care to a senior panel within a value-based care model. You will work with a dedicated care team to manage multiple chronic conditions and promote wellness across the patient journey.

Qualifications

• Active and unrestricted NP license.
• Current DEA registration.
• At least 2 years of outpatient primary care experience.
• Proficiency in minor office procedures.
• Experience in geriatrics is a plus, but not required.

Responsibilities

• Provide comprehensive primary care to a panel of up to 650 senior patients.
• Deliver high-quality outpatient services, managing multiple chronic conditions per patient.
• Work Monday through Friday, 8 a.m. to 5 p.m. (no weekends).
• Participate in a call schedule proportional to the number of providers.
• Collaborate with a dedicated care team including medical assistants, schedulers, and referral specialists.
• Utilize clinic resources to promote patient wellness and engagement.
• Perform office-based procedures (e.g., suturing, joint injections, skin biopsies, wound care).
• Support and promote on-site wellness activities for patients.
