Job summary

BoldAge PACE in North Charleston, SC seeks a PCP Nurse Practitioner to deliver primary care and ongoing healthcare services to PACE participants, developing care plans with the clinical care team. You will assess, reassess, monitor medications, coordinate with specialists, and participate in IDT meetings to ensure safe transitions and high-quality, participant-centered care.

The role requires state licensure, CDS and DEA, and BLS; prior geriatric or frail elderly experience preferred; this

Qualifications

• MD, DO, NP, APRN, or PA credential.
• Current State license.
• State CDS Certification and DEA Registration.
• Board Certified in Geriatrics preferred.
• BLS required within 90 days of employment.
• 2 years' experience providing direct patient care.
• 1 year of experience with frail elderly preferred.

Responsibilities

• Deliver high-quality primary care with individualized evaluation and history.
• Collaborate with IDT before referrals to specialists to ensure necessity.
• Develop and communicate participant care plans with clinical judgment.
• Manage medications and reconcile therapies with the Medical Director and Pharmacist.
• Coordinate transitions during hospitalizations and facility stays.
• Participate in daily IDT meetings and contribute to care plans.
• Ensure timely documentation and compliant coding for participants.
• Educate participants and caregivers on self-care and wellness.
