Qualifications
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Illinois.
- Minimum of 3 years of nursing experience.
- Minimum of 2 years of experience with geriatric patients.
- 2+ years of experience in Case Management or Care Coordination.
- Experience in post-acute or long-term care settings within the last 5 years.
Responsibilities
- Develop and manage comprehensive care plans for patients.
- Act as the primary contact and advocate for patients and families.
- Provide health education to improve understanding of chronic conditions.
- Conduct assessments to identify patients' health needs.
- Ensure medication adherence and compliance with follow-ups.
