Job summary
Cantex-Continuing-Care-Network is seeking a part‑time Nurse Practitioner in Houston to deliver holistic, patient‑centered primary care within skilled nursing and long‑term care settings. You will collaborate with physicians, nurses, and care teams to diagnose, manage, and prevent conditions while promoting wellness and quality of life.
The role requires adult/geriatric care experience, state NP/RN licensure, and strong care coordination skills.
Qualifications
- Active NP and RN licensure in the state of practice.
- Board certification (AANP or ANCC) is required or preferred.
- 2 years NP experience in adult/geriatric/family practice settings preferred.
- Strong ability to coordinate care and manage complex patients.
Responsibilities
- Perform comprehensive health risk assessments for assigned Members.
- Collaborate with physicians, RNs, case managers, social workers, and facility staff to coordinate care.
- Identify quality improvement opportunities and develop individualized care plans.
- Review and interpret medical, surgical, and social histories.
- Provide wellness counseling and prescribe appropriate treatments.
