Job summary

Cantex Continuing Care Network is seeking a part-time Nurse Practitioner to provide patient-centered care for adults and seniors in skilled nursing and long-term care settings.

The NP will collaborate with physicians, case managers, and interdisciplinary teams to manage acute and chronic conditions, perform risk assessments, and lead care coordination efforts, with a focus on post-acute care and value-based outcomes.

Qualifications

• 2 years of NP experience in adult/geriatric or family practice preferred.
• Strong adult/geriatric assessment and chronic disease management skills.
• proficient in medication reconciliation and clinical coordination.

Responsibilities

• Perform comprehensive health risk assessments for Members.
• Coordinate care with physicians, RNs, case managers, and staff.
• Develop individualized care plans and review histories.
• Prescribe medications and order tests within scope.
• Travel between facilities as required.
