Job summary

Cantex-Continuing-Care-Network is seeking a Part-Time Nurse Practitioner in Beaumont, TX, to provide holistic care for adult and geriatric Members across skilled nursing settings. You will work with physicians, RNs, and case managers to diagnose, manage, and prevent conditions while prioritizing wellness and quality of life.

This role blends onsite facility work with administrative duties, requiring travel between assigned Texas facilities and collaboration with interdisciplinary teams.

Qualifications

• 2 years of NP experience in adult/geriatric or family practice preferred.
• Strong ability to perform comprehensive adult/geriatric assessments and manage chronic disease.
• Skilled in identifying risk factors, unmet needs, and gaps in care for senior populations.
• Proficient in medication reconciliation and managing clinically complex patients.
• Graduate of an accredited NP program; board certification preferred.

Responsibilities

• Perform comprehensive health risk assessments for Members.
• Collaborate with physicians, RNs, case managers, and facility staff to coordinate care.
• Review and interpret medical, surgical, and social histories; develop care plans.
• Prescribe medications, order tests, and provide wellness counseling.
• Ensure HIPAA compliance and PHI protection; coordinate referrals as needed.

As a full‑time Nurse Practitioner, you will collaborate with physicians, RNs, case managers, and interdisciplinary care teams to diagnose, manage, and prevent acute and chronic conditions while promoting wellness and quality of life. This role includes accountability for Plan Medical Management, comprehensive health risk assessments, chronic disease management, and care coordination.
