Job summary

Cantex-Continuing-Care-Network is seeking a Part-Time Nurse Practitioner in Beaumont, TX, to provide holistic care for adult and geriatric Members across skilled nursing settings. You will work with physicians, RNs, and case managers to diagnose, manage, and prevent conditions while prioritizing wellness and quality of life.

This role blends onsite facility work with administrative duties, requiring travel between assigned Texas facilities and collaboration with interdisciplinary teams.

Qualifications

  • 2 years of NP experience in adult/geriatric or family practice preferred.
  • Strong ability to perform comprehensive adult/geriatric assessments and manage chronic disease.
  • Skilled in identifying risk factors, unmet needs, and gaps in care for senior populations.
  • Proficient in medication reconciliation and managing clinically complex patients.
  • Graduate of an accredited NP program; board certification preferred.

Responsibilities

  • Perform comprehensive health risk assessments for Members.
  • Collaborate with physicians, RNs, case managers, and facility staff to coordinate care.
  • Review and interpret medical, surgical, and social histories; develop care plans.
  • Prescribe medications, order tests, and provide wellness counseling.
  • Ensure HIPAA compliance and PHI protection; coordinate referrals as needed.

As a full‑time Nurse Practitioner, you will collaborate with physicians, RNs, case managers, and interdisciplinary care teams to diagnose, manage, and prevent acute and chronic conditions while promoting wellness and quality of life. This role includes accountability for Plan Medical Management, comprehensive health risk assessments, chronic disease management, and care coordination. The NP practices within the scope of state regulations, clinical protocols, and collaborative practice agreements, with a strong focus on post‑acute care, provider‑based care, and value‑based clinical performance.Qualifications• 2 years of experience as a Nurse Practitioner in adult, geriatric, or family practice settings preferred.• Strong ability to perform comprehensive adult/geriatric assessments and manage chronic disease• Skilled in identifying risk factors, unmet needs, and gaps in care for senior populations• Proficient in medication reconciliation and managing clinically complex patients• Graduate of an accredited NP program (Adult/Gerontology Primary Care preferred)• Active RN and NP licensure in the state of practice; board certified (AANP or ANCC)• Current CPR certification• Highly organized, detail‑oriented, and committed to ongoing learning and professional developmentPreferred• Geriatric certification (or willingness to obtain)• Bilingual (English/Spanish)• Familiarity with community resources and behavioral health needs• Experience serving vulnerable, low‑income, or disadvantaged populationsEssential DutiesPerform comprehensive health risk assessments for assigned MembersCollaborate with physicians, case managers, social workers, caregivers, and facility staff to coordinate careIdentify quality improvement opportunities and develop individualized care plansReview and interpret medical, surgical, and social historiesPerform appropriate screening procedures and interpret diagnostic dataRecommend and implement treatment plans, including prescribing medications, ordering tests, and providing wellness counselingReconcile medications and ensure safe, appropriate useRefer Members to PCPs, case management, or disease management programs as neededMaintain compliance with HIPAA, confidentiality standards, and PHI regulationsIdentify unmet needs and initiate appropriate referralsDiagnose conditions and create comprehensive, evidence‑based plans of careAchieve clinical performance targets and quality benchmarksBuild positive rapport and engagement with Members and familiesCommunicate and collaborate with facility leadership to support Member care goalsParticipate in committees related to clinical guidelines, quality improvement, and complianceMaintain team schedules to ensure continuity of careTravel independently to assigned facilities as requiredServe as a clinical leader within the facility, supporting provider‑based care and post‑acute care workflowsTravel RequirementsTravel required between assigned facilitiesMust meet company driving standardsMust maintain valid driver’s license and automobile insuranceMust be able to independently travel to all assigned locationsPhysical & Psychological DemandsProlonged sitting; occasional bending, stooping, kneeling, or stretchingManual dexterity to operate medical equipment and office technologyRequires close and peripheral vision, hearing, and ability to adjust focusAbility to lift and/or move up to 50 lbsAbility to follow universal precautions for blood‑borne pathogensModerate to intense concentration required due to clinical complexityWork EnvironmentCombination of administrative and clinical dutiesOccasional exposure to variable temperatures, airborne particles, and body fluidsModerate noise levelsReasonable accommodations available upon requestPlease visit cantexcc.com for more information about this location.We are an Equal Opportunity Employer.
