Job summary
CareMed Group in Kentucky seeks an experienced Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities. You will assess, diagnose, and manage chronic conditions, coordinate with an interdisciplinary team, and support care planning with physicians and staff.

Qualifications
- Active KY NP license required.
- Board-certified with DEA licensure.
- Experience in long-term care settings preferred.
Responsibilities
- Provide primary and preventive care for long-term care residents in skilled nursing facilities.
- Manage residents with acute changes in condition.
- Advanced care planning and chronic care management.
- Prescribe medications, including controlled substances per KY law.
- Order, review and follow up with treatments and diagnostics.
- Communicate with nursing home staff regularly about changes in condition and care plan.
- Collaborate with primary care physician.
