Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with geriatrics experience to serve residents in nursing homes or assisted living communities. You will collaborate with a team to deliver personalized, coordinated care and implement Treat in Place protocols under supervision where appropriate.

Ideal candidates have APRN/PA licensure and a track record in geriatrics care, with familiarity with fee-for-service settings and EHRs.

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal agencies related to clinical services you provide.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to treatment and modify plans as needed.
