Job summary

Essen Health Care is seeking motivated Nurse Practitioners and Physician Assistants to join our team serving multiple nursing home facilities across NYC. The position offers a supportive, patient-focused environment with pathways for new graduates to start their careers.

The Nurse Practitioner will conduct routine primary care and follow-up visits for residents in long-term care and subacute facilities, with training provided in New York before transitioning to New Jersey locations.

Qualifications

Master's in NP program: Family or Adult Ger geriatrics.
Current NY NP license and national board certification required.
Bilingual Spanish is a plus.
New grads encouraged to apply.
Experience with EMR/EHR is a plus.

Responsibilities

Provide primary care and follow-up visits for residents in long-term care facilities.
Coordinate with facility staff and supervising physicians.
Order/tests and interpret results to guide care.
Vaccinate patients and advise on prevention of disease.
Document patient information accurately in the medical record.
Deliver care in nursing home settings and collaborate with care teams.
Travel to multiple nursing home locations; training in NY before NJ locations.
