Qualifications

• Master's Degree in a Nurse Practitioner Program with a focus on Family or Adult Geriatrics.
• Current New York State Nurse Practitioner license.
• National board certification required (AANP/ANCC/ONCC).
• Spanish-speaking ability is a plus.
• EMR/EHR experience is a plus.
• New grads encouraged to apply.

Responsibilities

• Examin(es), diagnose(s) and treat(s) patients in long-term care facilities.
• Perform primary care visits and follow-ups for residents in nursing homes.
• Coordinate with facility staff and supervising physicians.
• Document medical histories, exams, medications and plans.
• Order and interpret labs and imaging as needed to guide care.
• Provide patient education on prevention and wellness.
