Job summary

Vitae Health Systems seeks a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. You will assess, diagnose, and treat emotional and behavioral issues, aiding residents' well-being during medical and life changes.

Responsibilities include psychosocial assessments, treatment planning, and individual/group/family therapy. License in state required; prior geriatric/long-term care experience preferred.

Qualifications

• Master's degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in the respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist).
• 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Knowledge of mental health conditions common in SNF populations (depression, anxiety, dementia).
• Proficiency in evidence-based therapeutic approaches (CBT, grief counseling).
• Ability to work collaboratively within an interdisciplinary team.

Responsibilities

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, grief, cognitive decline, and adjustment to illness.
• Support residents coping with chronic illness, pain, and loss of independence.
• Collaborate with the Vitae clinical team, attending physician, nursing staff, social workers, and rehab teams.
