Job summary

Vitae Health Systems is hiring a Psychotherapist to provide mental health services in a skilled nursing facility. You will assess, diagnose, and treat residents, offering individual, group, and family therapy to support well-being and adjustment to illness and life changes.

The role requires a Master’s degree and an unrestricted state license (IL). You will work with an interdisciplinary team, maintain HIPAA-compliant documentation, and participate in care planning.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted state license (e.g., LCSW, LCPC, LMFT, or Licensed Psychologist)
• 2+ years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings
• Strong clinical assessment, diagnostic, and treatment planning skills
• Knowledge of mental health conditions common in SNF populations (depression, anxiety, dementia)
• Proficiency in evidence-based therapeutic approaches (CBT, trauma-informed care, grief counseling)
• Ability to work collaboratively within an interdisciplinary team
• Excellent communication and interpersonal skills with residents, families, and staff
• Familiarity with electronic health records (EHR) systems preferred

Responsibilities

• Conduct psychosocial assessments for residents on admission and as needed
• Develop, implement, and monitor individualized treatment plans
• Provide individual, group, and family therapy sessions
• Address depression, anxiety, dementia, and adjustment disorders
• Collaborate with physicians, nursing staff, social workers, and rehabilitation teams
• Participate in care plan meetings and interdisciplinary sessions
• Maintain compliant clinical documentation
• Provide crisis intervention and behavioral management as needed
• Educate staff and families on mental health and communication strategies
• Ensure HIPAA compliance and ethical practice
