Job summary

Vitae Health Systems is seeking a qualified Psychotherapist to deliver mental health services to residents in a skilled nursing facility. You will assess, diagnose, and treat emotional and behavioral conditions, supporting residents' well-being and adjustment to medical life changes.

The role requires a Master's degree and an active state license. Collaboration with an interdisciplinary team and a focus on documentation and HIPAA compliance are essential.

Qualifications

• Master's degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in respective state.
• 2 years of clinical experience preferred in geriatric/long-term care or medical settings.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Knowledge of SNF mental health conditions (depression, anxiety, dementia).
• Proficiency in CBT, trauma-informed care, grief counseling.

Responsibilities

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, grief, and adjustment to illness.
• Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns.
• Collaborate with interdisciplinary team to coordinate care.
• Participate in care plan meetings and team discussions.
• Maintain accurate clinical documentation in compliance with regulations.
• Provide crisis intervention and behavioral management as needed.
• Educate staff and families on mental health and communication strategies.
• Ensure HIPAA compliance and ethical practice.
