Job summary

Carease Health in Paris, TN is seeking a Social Worker/Care Companion to support residents in skilled nursing facilities. The role focuses on emotional support, brief therapies, and mental health assessments within a Collaborative Care team.

You will visit facilities as scheduled, perform geriatric mental health evaluations, and provide short-term interventions while maintaining detailed records. Regular supervision and monthly psychiatry meetings are included.

Qualifications

• Degrees in social work, psychology, counseling or a mental health field.
• Geriatric mental health, crisis management, and psychosocial support in long-term care.
• Strong communication and documentation skills.
• Education: Associate Degree.
• Experience: 1 year.

Responsibilities

• Visit designated facilities on the schedule set with your supervisor.
• Assess residents to identify emotional, cognitive, and psychosocial needs.
• Provide short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain detailed records after each visit.
• Collaborate with psychiatric professionals, including monthly meetings with the psychiatric NP.
