Job summary

Carease Health is seeking a compassionate Mental Health Provider to join our Collaborative Care team, serving residents in skilled nursing facilities.

You will deliver emotional support, brief therapeutic interventions, and mental health assessments to older adults, with focus on empathy, listening, and culturally sensitive care. Regular supervision and collaboration with psychiatric providers are offered to support professional growth.

Qualifications

• Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health-related field.
• Geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• Education: Associate Degree

Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
