Job summary

Carease Health is seeking a compassionate Mental Health Provider to join our Collaborative Care team. You’ll visit skilled nursing facilities, provide emotional support, brief therapy, and mental health assessments to residents.

Ideal candidates have degrees in social work, psychology, counseling, or related field, and at least 1 year of geriatric mental health experience. Regular supervision and opportunities for professional growth are available.

Qualifications

• Degree in Social Work, Psychology, Counseling or related mental health field.
• Geriatric mental health knowledge and crisis management.
• Strong communication and documentation skills.
• Education: Associate Degree
• Experience: 1 year

Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
