Carease Health is seeking a compassionate Mental Health Provider to join our Collaborative Care team in Florida. You will deliver emotional support, brief therapy, and mental health assessments to residents in skilled nursing facilities.

This role focuses on improving residents' well-being through direct interaction, documentation, and coordinated care with psychiatric professionals. A supportive team environment and ongoing training are provided.

Qualifications

• Associates, Bachelor's or Master's degree in a mental health field.
• Experience in geriatric mental health and long-term care settings is preferred.
• Strong communication and documentation skills.

Responsabilités

• Visit designated facilities as scheduled with supervisor.
• Assess residents' emotional, cognitive, and psychosocial needs.
• Provide short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain records after each visit.
• Collaborate with psychiatric professionals and attend monthly meetings with the psychiatric NP.

Formation

Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health–related field
