The LISW/LPCC role involves conducting assessments, creating individualized care plans, collaborating with care teams, monitoring progress, maintaining confidential records, and delivering talk therapy using evidence-based approaches.

Qualifications

• Active Ohio license as LISW or LPCC (required).
• Passion for working with the elderly population.
• Ability to work independently within a facility setting.
• Strong communication and clinical documentation skills.

Responsibilities

• Conduct thorough assessments to understand clients’ needs, challenges, and mental health conditions.
• Identify individualized support options based on evaluations.
• Develop customized care plans for each client, considering their circumstances.
• Work closely with healthcare professionals, nursing staff, and case managers to coordinate services.
• Regularly assess clients’ progress and adjust care plans as needed.
• Keep track of treatment outcomes and make necessary modifications.
• Document treatments, interventions, and services provided.
• Engage in talk therapy using evidence-based techniques.
