Job summary

Health Here is seeking a Licensed Mental Health Counselor to provide compassionate behavioral health services to seniors in long-term care, assisted living, and community settings. The role includes onsite and telehealth psychotherapy, comprehensive assessments, and collaboration with interdisciplinary teams.

You will develop person-centered treatment plans, support residents with mood and anxiety concerns, and educate staff to strengthen behavioral health practices, ensuring holistic care

Qualifications

• Must hold a Licensed Mental Health Counselor credential (LMHC).
• Strong assessment, clinical documentation, and care-planning skills.
• Experience providing psychotherapy in both individual and group settings.
• Ability to work effectively in interdisciplinary and facility partnerships.
