Acclaim Health, affiliated with JPS Health Network, seeks a Board-certified Physician in Geriatric Medicine for outpatient care in Fort Worth, TX. You will manage a panel of geriatric patients, coordinate multidisciplinary care, and provide education and referrals as part of an integrated health system.

The role emphasizes compassionate, evidence-based care for aging populations, with opportunities to work across primary care, geriatrics and specialty services within a large public health

Qualifications

• Residency and fellowship in geriatrics from an ACGME-accredited program.
• Board-certified in ABFM or ABIM; CAQ in Geriatric Medicine considered within 5 years of joining.
• Current Texas medical license in good standing; unrestricted work authorization in the United States.

Responsabilités

• Coordinate multidisciplinary care for a patient panel to optimize outcomes.
• Review patient records, diagnose, treat, and provide care management and education.
• Make appropriate referrals and maintain collaboration with physicians, residents and AP professionals.

Formation

ACGME-accredited Internal Medicine or Family Medicine residency and Geriatric Medicine fellowship
