Job summary

Acclaim Health in Fort Worth, TX, seeks a Physician specialized in Geriatrics for outpatient care. You will manage an aging patient panel, coordinate multidisciplinary services, and provide education and care management in a hospital-adjacent, community-focused setting.

Applicants must hold an MD or DO with geriatrics training, be licensed in Texas or eligible, and be prepared to work in a collaborative environment with residents, trainees, and staff.

Qualifications

• MD or DO degree from an accredited school of medicine.
• Residency and/or Fellowship in Family Medicine with a geriatrics focus.
• Current Texas medical license; board certified or board eligible.
• Unrestricted work authorization to practice in the United States.
• Experience with electronic medical records is preferred.

Responsibilities

• Coordinate multidisciplinary care for the patient panel.
• Review medical records to diagnose, treat, and provide care management.
• Make appropriate patient referrals and ensure evidence-based care.
• Collaborate with other physicians, residents, and staff to deliver high-quality care.
• Support academic, educational and research endeavors where applicable.
• Provide empathetic, patient-centered care and engage with aging populations.
