Overview

Prisma Health, South Carolina's largest not-for-profit healthcare provider, is seeking a BC/BE Internal Medicine, Family Medicine, or Geriatrician (preferred) to join our SeniorCare PACE (Program of All-Inclusive Care for the Elderly) team in Columbia, SC.

About the Role

• Provide primary care for PACE participants, conducting assessments and managing medical needs.
• Work within a multidisciplinary team including nurses, social workers, therapists, dietitians, and specialists.
• Deliver care across various settings such as PACE centers, homes, hospitals, and nursing facilities.
• Perform geriatric assessments covering cognitive, functional, medication management, and end-of-life planning.
• Engage in care coordination and family discussions to align care with patient preferences.
• Collaborate with specialists and hospital teams for advanced care.
• Teaching opportunities available with an academic appointment at the University of South Carolina School of Medicine, based on experience.
• Knowledge of Medicare, Medicaid, and long-term care services is beneficial.
