Job summary

SCAN Health Plan is seeking a Medical Director to provide clinical leadership across a field-based care team from the San Fernando Valley to Pasadena. You will guide geriatric and palliative care initiatives, oversee physicians and nurse practitioners, and ensure high-quality, patient-centered care through home visits and telehealth.

The role emphasizes collaboration, performance metrics, and regulatory compliance, with opportunities to influence care strategies and mentor clinical staff.

Qualifications

• Geriatrics and palliative care experience required.
• Experience with virtual care/telehealth preferred.
• Demonstrated leadership and team coordination skills.
• Strong interpersonal and communication abilities.
• Knowledge of HIPAA and EMR documentation standards.

Responsibilities

• Directly manage a multi-disciplinary team of clinicians to ensure high-quality care.
• Provide primary medical care to patients in home/field settings or via telehealth.
• Lead quality improvement initiatives and monitor clinical performance metrics.
• Ensure accurate EMR documentation and compliance with CMS/Medicare guidelines.
• Engage in community stakeholder meetings and acts as clinical educator for staff.
