• Graduation from an accredited nursing education program.
• Experience in geriatric care with relevant courses and clinical practice.
• Licensed LVN in Texas with current license and MDS certification.
• Knowledge of general nursing theory and geriatrics.
• Familiarity with survey processes and state/federal guidelines.

Responsibilities

• Reviews preadmission applications for residency and assesses level of care when possible.
• Ensures admission status is documented within eight hours of admission.
• Completes the Minimum Data Set within fourteen days of admission.
• Implements actions for interdisciplinary health care plan within 21 days of admission.
• Assists in preparation for and during all surveys.
• Demonstrates nursing process knowledge and judgment in resident care.
• Coordinates rehabilitative and restorative care across residents requiring this support.
• Supervises rehabilitative aides and coordinates physician consultations and therapy.
• Acts as liaison between residents, families, and staff.
• Participates in Infection Control and Quality Assurance committees.
• Ensures health care plan goals are measurable and reflects resident wishes.
