Qualifications

• Active Licensed Vocational Nurse in California
• Fully vaccinated against COVID-19 including Booster shots
• Knowledge of long-term care patient needs

Responsibilities

• Complies with attendance policy requiring regular, reliable attendance.
• Participates in initial admission evaluation of residents and performs evaluations.
• Identify, manage, and treat skin disorders and lesions.
• Document pressure sores with size, stage, site, measurements, and drainage.
• Photographs pressure sores to document progression or improvement.
• Perform weekly body checks and consult with the DON as needed.
• Maintain pressure sore profiles and ensure prophylaxis and treatment.
• Provide pain medication prior to resident treatments.
• Collaborate with Care Planning Team on care plans.
• Initiate referrals and consults as needed.
• Respond to requests from residents, physicians, or nursing staff.
• Monitor and treat skin conditions per physician orders.
• Assist with dining program supervision as needed.
• Provide direct nursing care in emergencies and illness.
• Educate residents and family on skin care and treatment.
• Maintain confidentiality of resident care information.
• Ensure privacy and knock policy before entering rooms.
• Document and monitor wound care and preventive measures.
