Job summary

Rockville Center is seeking a Licensed Practical Nurse (LPN) to join our Skilled Nursing facility team in New York. The role requires current NY LPN licensure, the ability to evaluate and treat residents with minimal supervision, and a commitment to geriatric care.

The LPN will perform medication administration (including IVs where applicable), take vital signs, deliver resident care, provide family education, and collaborate with physicians, NPs, and CNAs. Alternate weekends required.

Qualifications

• Must hold NY LPN license.
• Experience with geriatric care is a plus.
• Alternate weekends required and shift is 7AM-3PM.

Responsibilities

• Provide care for residents in SNF/long-term care units.
• Medication administration, including IVs.
• Take and record vital signs and monitor outcomes.
• Provide resident and family education.
• Communicate with physicians, NPs, and health care teams.
