Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Licensed Clinical Professional Counselor to join our integrated clinical team onsite in local long-term care facilities. You will provide therapy, assessments, and care planning, delivering behavioral health support to older adults with chronic conditions.

This role offers a Monday–Friday schedule with robust admin and clinical support, enabling focus on patient outcomes and professional growth within a mission-driven

Qualifikationen

• Active, unrestricted LCPC license
• Strong written, verbal, and interpersonal communication skills
• Ability to work independently while collaborating with an interdisciplinary team
• Commitment to timely documentation and high-quality care

Aufgaben

• Clinical Care: Provide individual, family, and group therapy; conduct assessments and treatment planning; support residents with depression, anxiety, grief, trauma and other LTC-related conditions; deliver geriatric-focused behavioral interventions.
• Interdisciplinary Collaboration: Work with nursing, social services, rehabilitation, and medical staff; participate in care plan discussions and facility meetings.
• Documentation & Compliance: Maintain timely documentation using the Geriatric Notes Tool (GNT); adhere to ethical and regulatory guidelines.
• Facility Partnership: Build relationships with facility staff and leadership; serve as a behavioral health resource for residents.
