Job summary

Deer Oaks is seeking a Licensed Professional Counselor (LPC) to join our integrated clinical team onsite at one or more long-term care facilities in your community. The role provides therapy, assessments, care planning, and behavioral health support for LTC residents, with a predictable Monday–Friday schedule and strong clinical support.

Qualifications

• Active, unrestricted LPC license.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances.
• Deliver behavioral management and brief, goal-oriented interventions.
• Coordinate with nursing, social services, rehabilitation, and medical staff.
• Participate in care plan discussions and facility meetings.
• Maintain complete and timely documentation using GNT.
• Follow ethical, professional, and regulatory guidelines for documentation and care.
• Build strong relationships with facility staff and leadership.
