Job summary

Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Licensed Marriage and Family Therapist to join our integrated clinical team. You will work onsite in one or more long-term care facilities within your local community, providing therapy, assessments, care planning, and behavioral health support with strong administrative and clinical backing.

Qualifications

• Active, unrestricted LMFT license.

• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, grief, adjustment to illness, and other LTC-related conditions.
• Deliver behavioral management and brief, goal-oriented interventions.
• Collaborate with nursing, social services, rehabilitation, and medical providers.
• Participate in care plan discussions and facility meetings.
• Maintain complete and timely documentation using the Geriatric Notes Tool (GNT).
• Ensure confidentiality and compliance with regulatory guidelines.
• Build strong relationships with facility staff and leadership.
• Serve as a behavioral health resource for residents.
