Job summary

Deer Oaks is seeking a Licensed Mental Health Counselor to join the clinical team onsite at one or more long-term care facilities in your local community. You will provide therapy, assessments, care planning, and behavioral health support with strong administrative and clinical backing.

Qualifications

• Active, unrestricted LMHC license required.
• Strong written, verbal, and interpersonal communication.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Collaborate with nursing, social services, rehabilitation, and medical providers to coordinate treatment.
• Maintain complete and timely documentation and ensure HIPAA-compliant practices.
