Job summary

Highland Pines is seeking a Licensed Bachelor's Social Worker (LBSW) in Texas to join our long-term care team. You will identify psychosocial, mental, and emotional needs for residents and help facilitate access to services in accordance with NASW Code of Ethics and applicable regulations.

The role requires Texas licensure, a social work degree, and at least one year of supervised experience in healthcare with geriatric clients; nursing experience in SNFs is preferred.

Qualifications

• Licensed or registered in the state of practice (Texas)
• Bachelor's in Social Work or Human Services required
• 1 year of supervised social work experience in a healthcare setting with geriatric individuals
• 2+ years nursing experience in a Skilled Nursing Facility preferred

Responsibilities

• Perform social history and psychosocial assessments to identify needs and guide the care plan
• Maintain accurate documentation per federal/state regulations and center policies
• Support resident rights and inform about therapeutic alternatives when needed
• Contribute to interdisciplinary care planning and interventions for psychosocial needs
• Assist with discharge planning and referrals to home care services when appropriate
• Collaborate with nursing and administration to promote resident well-being and comply with inspections
