Job summary

Focused Post Acute Care Partners is seeking a licensed Social Worker in Beaumont, TX to coordinate social services for residents. You will interview and refer residents to community resources, advocate for residents' rights, and collaborate with the care team to support emotional well-being.

The role requires a Texas license or eligibility within 6 months, a Bachelor's in Social Work or related field, and experience in long-term care or hospitals. Emergency response ability is preferred.

Qualifications

• Bachelor's Degree in Social Work or Human Services field.
• Must be a licensed Social Worker in Texas or obtain within 6 months of hire.
• Prefer two years of experience in long-term care, hospital or related medical facilities.
• Able to react to emergency situations appropriately.

Responsibilities

• Meet with administration, medical and nursing staff to plan social services.
• Advocate daily on behalf of residents to ensure their rights are maintained.
• Reports abuse, neglect, or exploitation per state guidelines.
• Maintains professional working rapport with facility interdisciplinary team and community resources.
• Completes Social Service History and Social Service Evaluation with newly admitted residents within 14 days.
• Completes sections B, C, D, E and Q of MDS 3.0 on days 5, 14, 30, 60, 90, annually, upon change of condition, and upon resident readmission from hospitalization.
