Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care to older adults in their homes or senior living communities in Raritan, NJ.

Evaluations, individualized plans, and evidence-based treatments are provided, with documentation in FOX EMR and collaboration with an interdisciplinary team. Telehealth options are available to extend access and flexibility.

Qualifications

• Valid Physical Therapy license in the state of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer skills and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that promote mobility, independence and functional goals.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists to coordinate care.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth or virtual care when appropriate to extend access.
