Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist for the Sherborn, MA area. This full-time outpatient PT will serve geriatric clients in senior living communities, providing evidence-based therapies to improve function and independence.

No prior experience is required and new graduates are encouraged to apply. You will be part of a multidisciplinary team delivering outpatient services to help clients age in place.

Qualifications

• Massachusetts Physical Therapist license is required.
• Experience in outpatient geriatric care within senior living communities preferred.
• New grads are encouraged to apply.

Responsibilities

• Make outpatient visits to clients in senior living communities.
• Perform mobility, strength, balance, and safety assessments.
• Develop and implement individualized care plans including exercises and gait training.
• Continuously revise the plan of care and participate in multidisciplinary meetings.
• Educate clients, families, and community staff on therapy services to promote independence.
• Accurately document observations and interventions in the chart.
