Job summary

Halcyon Home LLC is seeking a Home Health Physical Therapist for a PRN position in the Spring, TX area. You will assess patients, develop and adjust plans of care, and educate patients and families in their homes.

The role involves supervising staff, coordinating with physicians, and ensuring compliance with agency standards. A current PT license and experience in home health are required.

• Current PT license.
• Experience in home health or related healthcare setting.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Perform ongoing clinical assessments, including OASIS assessments, to identify patient needs and determine level of functioning.
• Develop and evaluate plan of care in partnership with patient, representatives, and caregivers.
• Document clinical notes, phone orders, and other records on an ongoing basis and in a timely manner.
• Provide services ordered by the physician as indicated in the plan of care and oversee other team members when applicable.
• Apply infection control and standard precautions in patient care activities.
• Accept clinical assignments consistent with education and competence to care for patients.
• Provide supervision and guidance to physical therapy assistants and home health aides.
• Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
• Communicate with all physicians and other health care practitioners involved in the plan of care.
• Meet mandatory continuing education requirements of the Agency and licensing board.
• Assist and instruct patients and families on transfer techniques and use of equipment such as walkers, wheelchairs, and canes.
• Provide patient, caregiver, and family counseling and education.
