Job summary

Fox Rehabilitation is expanding its Geriatric House Calls™ program in New Jersey. We seek licensed Physical Therapists to deliver one-on-one care to older adults in private homes and senior living communities.

Expect autonomy, flexible scheduling, and meaningful patient outcomes in a home-based model. Clinicians may incorporate telehealth and other virtual care formats to extend access while maintaining high-quality, individualized treatment plans.

Qualifications

• Valid Physical Therapy license in NJ or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer skills and EMR proficiency; CPR certification current.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and safety.
• Document within the EMR system in compliance with regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
