A healthcare provider in California is seeking a Primary Care Physician to deliver high-quality coordinated care for seniors. Ideal candidates should have an active California medical license and be Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics. Qualifications

• Active MD or DO license in California is required.
• Experience caring for seniors or complex patients is preferred.
• Comfortable in a team-based, home-centric care model.
