Monarch Healthcare Management seeks a Clinical Dietitian to provide nutrition services to residents in a long-term care setting, collaborating with the multidisciplinary team to achieve patient-centered goals through the Nutrition Care Process.

Responsibilities include comprehensive nutrition assessments, PES statements, care planning, education, monitoring, and ensuring regulatory compliance, with opportunities for professional growth within Monarch’s network of facilities in Minnesota.

Qualifikationen

• Bachelor’s degree in dietetics, foods and nutrition or related area from a U.S. regionally accredited college or university.
• Completion of a didactic program in dietetics and supervised practice program approved by CADE of the ADA.

Aufgaben

• Apply the Nutrition Care Process and medical nutrition therapy for geriatric residents.
• Conduct and document thorough nutrition assessments and complete MDS requirements.
• Develop PES statements and individualized nutrition care plans.
• Provide nutrition education and counseling to residents and families.
• Monitor outcomes and coordinate with physicians and care teams.
• Ensure compliance with HIPAA, CMS, and state regulations.
