Job summary

Monarch Healthcare Management in Minnesota seeks a Clinical Dietitian to provide nutrition services to residents in a long-term care setting. You will work with the multidisciplinary team to deliver care aligned with patient goals and standards.

Responsibilities include applying the Nutrition Care Process, performing assessments, developing care plans, educating residents and families, monitoring progress, and ensuring HIPAA compliance. Experience in geriatric nutrition is preferred.

Qualifications

  • Bachelor’s degree in dietetics, foods and nutrition, or related area.
  • Completion of a didactic program in dietetics and supervised practice program approved by CADE.
  • Must be registration eligible and licensed within 1 year of employment.
  • Experience in healthcare, skilled nursing facility or assisted living community 1-2 years preferred.

Responsibilities

  • Apply the full Nutrition Care Process and medical nutrition therapy for geriatric residents using strong clinical judgment, counseling skills, cultural competence, critical thinking, and effective written and verbal communication.
  • Conduct and document thorough nutrition assessments, complete MDS requirements, evaluate medical, psychosocial, functional, and behavioral factors, supervise data collection, and identify nutrition-related risks requiring intervention or referral.
  • Analyze assessment data to identify and prioritize nutrition diagnoses, document PES statements, and develop evidence-based, individualized nutrition care plans tailored to resident safety, needs, and preferences.
  • Implement nutrition interventions, provide resident and family education and counseling, coordinate care with interdisciplinary teams, delegate appropriately, and adjust plans based on resident response and treatment goals.
  • Monitor outcomes through meal observations, progress tracking, and outcome measures; communicate with physicians and care teams; document care accurately; support discharge planning; and serve as a nutrition resource to staff, residents, and families.
  • Ensure compliance with HIPAA, CMS, state regulations, and accreditation standards; promote a positive, inclusive workplace; assist with surveys, audits, student education, and operational support to enhance quality and safety.

Clinical Dietitian
Position Summary

The Dietitian provides nutrition services to residents living in a long-term care setting and works closely with the multidisciplinary healthcare team to provide patient care that is integrated and compatible with the patient-focused medical goals and objectives. Primary responsibilities include application of the Nutrition Care Process (NCP) including completion of the nutrition assessment, determination of the nutrition diagnosis, developing and implementing nutrition intervention(s), and monitoring and evaluating of the patient’s progress. The Clinical Dietitian uses the Scope of Dietetics Practice Framework (SODPF) to determine scope of practice, established Standards of Practice and Professional Performance (SOP/SOPP) in nutrition care and practice-specific dietetics, and as available, evidence-based practice protocols/nutrition practice guidelines to help determine nutrition intervention(s).

Qualifications
  • Must have a bachelor’s degree in dietetics, foods and nutrition, or related area, granted by a U.S. regionally accredited college or university.
  • Completion of a didactic program in dietetics and supervised practice program approved by the Commission on Accreditation for Dietetics Education (CADE) of the American Dietetic Association.
  • Must be registration eligible and licensed within 1 year of employment.
  • Experience in healthcare, skilled nursing facility or assisted living community 1-2 years preferred
