Job summary

Monarch Healthcare Management is seeking a Clinical Dietitian to provide nutrition services to residents in a long-term care setting, collaborating with the multidisciplinary healthcare team to deliver patient-focused care aligned with medical goals.

Responsibilities include applying the Nutrition Care Process, conducting comprehensive nutrition assessments, diagnosing nutrition problems, planning and implementing interventions, and monitoring progress while ensuring HIPAA and CMS compliance.

Qualifications

• Bachelor’s degree in dietetics, foods and nutrition, or related area.
• Didactic program in dietetics with CADE approval.
• Registration eligibility and licensure within 12 months of employment.
• Healthcare experience in a skilled nursing facility or similar setting (1–2 years) preferred.

Responsibilities

• Apply the Nutrition Care Process and medical nutrition therapy for geriatric residents with clinical judgment and communication skills.
• Conduct thorough nutrition assessments, complete MDS requirements, and evaluate medical and psychosocial factors.
• Analyze data to identify PES statements and develop individualized care plans based on safety and resident preferences.
• Implement nutrition interventions and educate residents and families, coordinating with the interdisciplinary team.
• Monitor outcomes through observations and documentation, support discharge planning, and act as a nutrition resource.
• Ensure HIPAA, CMS, state regulation compliance and assist with quality improvement initiatives.
