Job summary

IVYPARK seeks a registered dietitian or RD/RDN to lead clinical nutrition in our skilled nursing and rehabilitation setting in Pittsburgh. You will perform comprehensive nutrition assessments, manage high-risk residents, and collaborate with the interdisciplinary team to optimize care and outcomes.

The role requires licensure, CDR certification, and 1–2 years of LTC experience; familiarity with PointClickCare or MatrixCare and MDS entries is preferred.

Qualifications

• Must be a RD/RDN certified by CDR.
• Hold a current state license or certification as required by the state of practice.
• 1–2 years of clinical LTC/sub-acute or skilled nursing experience preferred; new grads with geriatrics rotations considered.
• Proficiency in calculating enteral nutrition regimens and identifying signs of malnutrition.
• Experience with long-term care software and MDS entries.

Responsibilities

• Perform comprehensive nutrition assessments at admission, quarterly, annually, and during significant changes in status using standardized tools (e.g., MDS 3.0 K).
• Develop, implement, and monitor interventions for high-risk residents (weight loss, enteral feeding, pressure injuries, dialysis).
• Participate in weekly interdisciplinary care team meetings with nursing, SLP, physicians, and PTs.
• Ensure timely and accurate documentation of nutrition status and interventions within the EHR and MDS.
• Collaborate with Dietary Manager to review menus, ensure liberalized diet plans meet nutritional adequacy, and maintain sanitation and food safety standards.
• Maintain facility readiness for CMS surveys by aligning practices with CMS F-Tags.
