Job summary

BoldAge PACE is seeking a registered dietitian to assess nutritional status of participants, plan interventions, and collaborate with the Interdisciplinary Team to meet the nutritional needs of the participants. The role includes providing nutrition counseling, education to families, and participating in care planning to coordinate 24-hour care delivery.

Qualifications

• Bachelor’s degree in food, nutrition, or dietetics.
• Passing the CDR exam.
• Minimum 1 year working with the elderly; training provided if needed.
• Minimum 2 years clinical nutrition experience preferred.
• NJ: RD registration eligibility or active registration.

Responsibilities

• Provide high quality nutritional care as a member of the IDT.
• Conduct initial comprehensive nutritional assessments at enrollment.
• Develop care plans with the interdisciplinary team and revise as needed.
• Provide nutritional counseling to participants, caregivers, and family.
• Facilitate nutrition-related care plan interventions and home meals.
• Monitor weight, diagnoses, hydration, and report changes with recommendations.
• Educate other health professionals on nutrition and participate in IDT meetings.
• Incorporate cultural and ethnic preferences into care plans.
