Job summary

Phoenix of Maple Heights is seeking a PRN Occupational Therapy Assistant (COTA) to join our dynamic therapy team. This role focuses on geriatric care within a Skilled Nursing/LTC setting in Maple Heights, OH, with opportunities to work across two facilities.

You will document each session in Net Health, collaborate with an interdisciplinary team, and contribute to resident goals while maintaining regulatory compliance. New grads are welcome to apply and be part of a supportive environment.

Qualifications

• Active Ohio Certified Occupational Therapy Assistant license required.
• SNF/LTC experience preferred, new grads welcome.
• Excellent communication and time-management skills.

Responsibilities

• Implement OT treatment plans under supervision of an OT.
• Provide skilled, resident-centered OT services.
• Document treatments, progress notes, and outcomes in Net Health.
• Collaborate with therapy staff and interdisciplinary team.
• Maintain compliance with state and federal regulations.
