Job summary

Majestic Care of South Bend is seeking a Certified Occupational Therapy Assistant (COTA) to support high-quality rehabilitative care under an Occupational Therapist. This role focuses on implementing individualized treatment plans to enhance residents’ ability to perform ADLs, improve mobility, and promote independence.

You will collaborate with an interdisciplinary team in a geriatric SNF, document accurately, and uphold HIPAA and safety standards in daily practice.

Qualifications

• Associate’s degree from an accredited Occupational Therapy Assistant program.
• Current state licensure as a Certified Occupational Therapy Assistant (COTA).
• 1-2 years of experience in a skilled nursing facility or geriatric care setting preferred.
• Familiarity with PDPM and SNF documentation standards is a plus.

Responsibilities

• Deliver occupational therapy interventions as outlined in the treatment plan by the supervising OT.
• Assist residents with therapeutic exercises and ADLs to improve independence and mobility.
• Monitor resident responses during treatment and communicate progress or changes to the OT.
• Support the use of adaptive equipment and provide instruction to residents and caregivers for safe use.
• Maintain a safe, engaging therapeutic environment to foster participation and comfort.
• Accurately document sessions, progress, and observations per facility policies and regulations.
• Adhere to HIPAA, infection control, and OSHA standards.
