Job summary

Majestic Care of South Bend is seeking a Certified Occupational Therapy Assistant (COTA) to support rehabilitation for residents in a geriatric setting. Under the supervision of an Occupational Therapist, you will implement treatment plans to improve ADLs, mobility, and independence. Join a team that emphasizes compassionate, resident-centered care, accurate documentation, HIPAA compliance, and collaboration with an interdisciplinary staff. Prior SNF experience and PDPM familiarity are a plus.

Qualifications

• Associate’s degree (COTA) and current state licensure
• 1–2 years of experience in a skilled nursing facility or geriatric care setting preferred
• Familiarity with PDPM and SNF documentation standards is a plus

Responsibilities

• Deliver OT interventions as outlined in the plan developed by the supervising OT.
• Assist residents with therapeutic exercises and ADLs to improve independence and mobility.
• Monitor responses and report progress or changes to the OT.
• Support adaptive equipment use and provide instruction to residents and caregivers.
• Maintain a safe, engaging therapy environment and participate in care planning meetings.
